Alberta Health cannot update your vaccine records from this request – instead this data must be submitted to Alberta Health by the immunization provider.

It may take up to 10 business days to respond to your request. You may be contacted by our support agent using the contact provided on your submitted form if further information is needed, or to notify you once the investigation is complete.
